The quicktime play of a voicemail, only comes out through the speakerphone speaker. I should be able to listen to a message without broadcasting it- through the ear speaker.

This is a documented limitation of web applications on the iPhone. Audio on web sites do not play through the phone's earpiece, but will play through headphones (including Bluetooth devices) if connected.
